To calculate the modifier from an ability score, subtract 10, divide by 2, then round down.
(13-10)/2 = 1.5, so the modifier for a score of 13 is +1.
(18-10)/2 = 4, so the modifier for a score of 18 is +4.

In addition to the above, the correlation is, if you'll forgive my being blunt, painfully obvious.
Your Strength modifier is defined by your Strength score. Your Dexterity modifier by your Dexterity score. Etc.
As such, there are only six ability score modifiers. Other bonuses, penalties and modifiers you have for die rolls are not ability modifiers, they're other kinds of bonuses/penalties. _________________ Oy, this is going to be troublesome *sigh* |
